Nancy Leveson: Professor of Aeronautics and Astronautics, Massachusetts Institute of Technology

Nancy Leveson is Professor of Aeronautics and Astronautics at MIT. Previously, she was a professor in the Computer Science Departments at the University of Washington and the University of California, Irvine. She has published over 200 research papers and is author of two books: Safeware: System Safety and Computers (1995) published by Addison-Wesley and recently translated into Japanese and Engineering a Safer World (2012) published by MIT Press and currently being translated into Japanese and Chinese. She has served as Editor-in-Chief of IEEE Transactions on Software Engineering. Dr. Leveson consults extensively in many industries on the ways to prevent accidents and has served on numerous national and international committees and accident investigations including being an expert consultant for the Columbia Accident Investigation Board, the Presidential Commission on Deepwater Horizon, the Baker Panel on the Texas City accident, and a Navy committee investigating one of the V-22 Osprey accidents involving software as well as lesser-known accidents. 
 
Prof. Leveson conducts research on all aspects of system safety including design, operations, management, and social aspects. While she started in computer science and software engineering, she has migrated to the larger field of system engineering and subfields of particular importance such as system and software safety, requirements engineering, system analysis, human factors, human–automation interaction, and organizational safety. Dr. Leveson works in and her techniques are applied in a wide variety of safety-critical industries including aerospace, transportation, chemical plants, nuclear power, medical devices, and many others. One particular common element throughout all her work is an emphasis on applying systems thinking to complex systems. Prof. Leveson has degrees in mathematics, computer science, and management along with graduate study in developmental and cognitive psychology.

Anna Collard-Scruby: Human Factors Programme Specialist, Civil Aviation Authority

Anna Collard-Scruby is a Chartered Occupational Psychologist, and joined the CAA in May 2018 as the Human Factors Programme Specialist. Her previous experience includes three years with the Human Factors team at NATS Prestwick (Scotland), and seven years serving as a Psychology Officer with the Royal New Zealand Air Force.  Anna has worked across a broad range of aviation psychology and human factors activities including; human performance analysis, performance coaching, personnel selection and training, interrogating research, safety data analysis and classification, change management, unit development, resilience and wellbeing, military deployment support, and human factors advice and assurance.

Chris Quick: UK HPR1000 GDA Design Lead Project Correspondent, General Nuclear System (on behalf of EDF Energy)

Chris Quick is a Fellow of the IMechE who has worked in the Nuclear Industry for over 10 years, across both defence and civil sectors.  He has a strong background in both safety case and plant engineering/design and has worked across a range of reactor technologies in both the UK Generating Fleet and in the Nuclear New Build arena.  In his most recent role Chris worked in General Nuclear System, a Joint Venture Company formed between EDF Energy and China General Nuclear to deliver the Generic Design Assessment for the Chinese HPR1000 technology.  As Lead Project Correspondent for Design, Chris was accountable for a range of aspects including a number of key Engineering assessment areas and their associated safety cases (Civil Engineering, Instrumentation and Controls, Conventional Health and Safety, Conventional Fire Safety), as well as implementing the design management arrangements for the project.  As a leader in the Nuclear Industry Chris is passionate about working with people and developing them and teams they work in.

Mark Boult: Director, DNV GL

Mark Boult is highly experienced in providing in risk management and analysis support to clients in the energy, chemical and petrochemicals, offshore oil and gas, maritime, utility, healthcare and transport industries. He has held a range of management and consulting positions during a career of over 30 years and has international experience through worked in six of DNV GL’s offices around the world, namely London (UK), Stavanger (Norway), Columbus (Ohio), Houston (Texas), Hong Kong and Sydney (Australia).

In addition to leading and developing business for DNV GL, Mark has been involved in the development of new and innovative services and solutions for DNV GL’s clients. This has included input to the development of DNV GL’s risk management software products and ERM (enterprise risk management) services (including working with boards and management teams on their strategic risks), customising management systems solutions, asset management services (RBI and ARM), risk communications, and creating innovative solutions for DNV’s clients drawing on skills of DNV personnel from across the organisation.

Currently Mark is a Director in DNV GL's UK operations with a specific role on continuing the development and growth of the business, building customer relations, and oversight of the organisations ERM services in the region.

Anne Davies: Partner, Gunnercooke LLP

Anne Davies is a Business crime specialist. She has handled a broad range of regulatory investigations over the last 20 years, assisting both companies and individuals to minimise the risk facing them through criminal and civil liabilities arising from internal or external investigations. She has the knowledge and resources to work on domestic and international business crime litigation matters including bribery and corruption, environmental crimes; health and Safety; product safety; white collar crime.
Anne is particularly well known for her ability to advise Boards of Directors. She has developed a range of training programmes and tools to help Directors understand their responsibilities, and understand how to deal with crisis situations should they occur. She runs an innovative training programme which involves coaching Directors undergoing a simulated interview under caution, so participants can experience the reality of an external criminal investigation. Clients and peers praise her practical and proactive approach in providing this Board-level advice.

Keith Miller: Member, Institution of Mechanical Engineers’ Safety and Reliability Working Group

Keith has 45 years of experience in the oil and gas industry, with over 30 responsible for safety cases for Shell UK.  He is responsible for many innovative projects that have improved safety and business competitiveness.  He is a passionate advocate of the well-reasoned argument and has published work debunking probabilistic processes.

Mark Gabbott: Principal Nuclear Safety Inspector, Office for Nuclear Regulation

Mark Gabbott is a Principal Nuclear Safety inspector and has worked for the Office for Nuclear Regulation for 13 years, and is presently the Nominated Site Inspector for Sellafield. Mark has undertaken a variety of other work within the ONR, including Generic Design Assessment of new reactor designs, delivery of regulatory core training in his role as Nuclear Training Manager, and project inspection and mechanical engineering assessment within the nuclear defence sector . Prior to this Mark worked in the rail industry for over 10 years, as a mechanical engineer and design manager in the electrification field, for Balfour Beatty Rail Projects Limited. Mark also worked for BNFL in the Risley design office in his early career, and worked for a short period as a college lecturer in physics, electronics, and engineering. Mark is a Chartered Mechanical Engineer.

Jonathan Pyke: Human Factors Specialist, Sellafield Ltd

Jonathan Pyke leads the Human Factors team at Sellafield, he has worked in HF on site for 10 years, leading the team for the last 3. His academic background is Psychology.  His primary interest is safety critical operational control measures, their designation, implementation and substantiation.

David Dowdall: Human Factors Specialist, QinetiQ UK

David Dowdall has been a member of Aircrew Systems Integration team at QinetiQ UK since 2013, where he conducts Human Factors Test and Evaluation activities as part of Independent Technical Evaluation and Safety Assessment tasks. He has predominantly worked in the Aviation Domain, supporting the UK Ministry of Defence in understanding the risk associated with operating its Helicopter, Multi-Engine and Fast Jet fleets. In his role, David employs both analytical methods, such as Human Reliability Assessment tools, and practical aircraft trials.
