- Why humans are vulnerable to making errors in a modern work environment
- Limitations to the way we perceive the world around us
- Avoiding false conclusions about human error ‘causing’ negative events
- Factors which exacerbate or reduce the likelihood of making an error
- The inevitability that errors will always occur and cannot be totally eradicated

Karen Stevenson: Compliance Officer, GSK
Karen has worked in the pharmaceutical industry for over two decades, across dosage forms, oral to sterile. Ranging across quality, GxP and regulatory affairs; engineering, automated systems and process; in roles from GxP Auditor to Validation Manager and head of department.
She is currently working for GSK within GMS Engineering and EHS global compliance.

Karen is Chair of the Pharmaceutical TAC committee and has been involved with the institution for over 15 years.
